The ActiveX control implements an IMozControlBridge interface with a single method - GetWebBrowser for this purpose. After creating the control normally, QI for this interface and call the method to get ahold of the nsIWebBrowser XPCOM interface. You can then start hitting the Gecko APIs directly from there though you'd have to be careful not to tread on the control's toes, e.g. by unexpectedly releasing things etc.
