I have exactly the same problem sometime when another big application is
running in the background or one that uses 100% cpu like rendering or
seti. In my machine mozilla is then present in the task manager but do
not shows up.
Solution: kill the running mozilla in the task manager, stop the other
big program (if any) and try again.
Sorry no other suggestion
