Our old Cable Modem ISP (Armstrong Zoom Internet) used some sort of transparent upstream cacheing. Even though the computers were configured as if they had a "direct" internet connection, sometimes old web pages would be retrieved. Slashdot would do this very frequently.
I remember on occasions I would try to retrieve a file with my web browser and it would retrieve an old version. I'd VNC into my home PC (on RoadRunner), and attempt to retrieve the very file.... and it's the correct (new) version! I'm sure there are other causes for old pages to be retrieved, but to anybody seeing this problem perhaps consider your ISP.
