Ah, http://www.w3.org/TR/CSS21/page.html#page-break-props mentions that when we break between block frames, the bottom margins before the break and the top margins after the break are zeroed out. That suggests that when a block breaks internally, its top margin should only be applied to the first in flow and the bottom margin to the last in flow.
