Personal Toolbar

Can anyone tell me how to add an item to the Personal Toolbar?  I 
haven't downloaded any nightlies in a longtime.  When I did, I noticed 
that Bookmarks has a new look and feel.  Or should I say, is managed 
differently.  What gives?
