I have had the same problem. Can you clarify something? Does the tool bar itself appear and it is just your own entries that are missing from it? If so, is there a Personal Tool Bar entry on the Bookmarks menu, and does it expand to show all your PTB links? If you open bookmarks.html in a text editor can you see your PTB entries? If the answers are yes then, in your profile directory, delete (or rename) localstore.rdf, XUL.mfl, and panacea.dat when Moz is not running.
