Surely you mean "installed-chrome.txt" ?
Also, make sure you have a newline at the end of the installed-chrome.txt file... the parser is a bit finicky.
All your other steps look right; can you load your toolbar directly by loading chrome://notetaker/content/browserOverlay.xul ? (no XML parsing errors, and at least loads?)
