I can't speak for why the repairs didn't hold up, unless they were
temporary patches that should have been properly repaired.

I saw a lot of potholes in San Diego in March this year.  Certainly not
due to freeze/thaw cycles.

As far as technical information, it's probably there in more detail than
you would like.  I know that there are a few recent books in the Hennepin
library system and probably in Minneapolis as well.  Do a subject search
for either HIGHWAY ENGINEERING or TRAFFIC ENGINEERING.  Typically 3-4
inches thick and heavy to boot.  Keep to the newer ones.  There is also
likely to be some stuff on MNDOT's web site talking about experiments in
longer lasting pavement.

A few year ago, I read an article that contrasted the way highways are
built in Germany with the US.  Much heavier duty in Germany.
