One of the biggest problems is lack of follow up after someone is released. My guess is he was not med compliant.
There's alot of problems here -- not just the shooting, but how the mental health system allowed this guy to get to this state. There's some negligence there. There really needs to be better follow up care when folks are released from the hospital. Some of what I think would be good is a visiting nurse to help with monitoring medications, working with the families, so families can identify early warning symptoms to get the person help before things reach a crisis point. If the appropriate procedures were followed in this instance, then possibly procedures need to be changed.
