Do you suppose the system may be subject to abuse - false or confused allegations which only result in confusion or some form of harrassment?
An alternative for some situations: If bad judgement is used by a car driver or bike rider or pedestrian, why not invite that person for a cup of coffee, tea, or for a DQ or something. While the problem may be mentioned at some point, be very clear that this you would like to go beyond that a bit, to simply spend time being a respectful neighbor or fellow-metro-citizen, or fellow human. It is not even necessary to "be right" or "make a point" or try to make anyone recant, repent, or any such thing.

Cars have greatly increased our speed. So much so that we need to rush in order to try to keep up with the schedule we have imposed over our lives by virtue of supposedly "liberating" technology. we are bound by our speed, and so live less in the human dimension, and more in-humanely.
Hence the need for anonymous allegations sent through the mail by the state, from/to people who do not know one another.
We need to create urban infrastructure and urban transportation that is as human-scale and human-speed as possible.
And we need to slow down. We need to make time and space in our hearts as well as our outer infrastructure - time and space to transform conflict into grace-filled moments of humane connection.
Life is too short for anything less.
(D-e-e-e-e-e-e-e-p Br-e-a-t-h-i-n-g can also change us and our city. And it is cheap, too!)
