Probably because smoking is primarily a phenomena of the lower social-economic classes. The more people are poor, less educated, unemployed or in unskilled employment, etc., the more likely they are to smoke.Why do you think the ban is elitist?
With all the scientific and emotional arguments raging back-and-forth, elitism is a new one.

So a smoking ban inside businesses is most annoying to those people. But the proponents of this ban are on average more middle-class, more educated, etc. compared to smokers. So I guess you call that elitist.
