First, look at http://people.ee.ethz.ch/~oetiker/webtools/mrtg/config.html (the threshhold checking part) to familiarize yourself with that.
When a threshold is reached a command is executed. The command can be anything, a batch file, a program, etc.... I run a batch file that sends me an email using Blat: http://www.interlog.com/~tcharron/blat.html Install blat, test it out and then create a batch file to send you an email. Once your batch file is working, use it in your mrtg config file to execute when a threshold is reached.
