You need to specify what kind of server. Speaking on Windows platforms, there are a few ways to do this with MRTG. You could use Paul Simmonsons' excellent Getlog.pl script which uses performance monitor to create text files which are then processed by an intermediary file to create the MRTG log files, or you could used an SNMP agent, either the ones that come with Windows (somewhat limited), or try the SNMP Informant agent from www.snmp-informant.com.
