The short answer is an emphatic Yes.
But you'll probably need a bit of VB code to "set" the option buttons you
would need to use to emulate the look of the form.

I just uploaded a pdf called "Form 58XX (13262 - Draft, Tradi". It is a
draft copy of form that we use to capture deer harvest data. Successful
hunters will complete this form for each deer harvested. Data are
scanned from this form and ultimately dumped to an Access table.
As the database manager, I handle all of the information requests that
come from both outside and within the agency. Typical requests are the
form - did John Doe kill a deer and if so, can you send me the details
assoicated with each deer harvested. When a match is found, and
e-transfer is an option, I usually just cut and paste the relevant data
directly into an email or Excel spreadsheet and send it to the person.
If a hard copy is requested, I do the same and print the information
out.
My question for the group is this - would it be possible to create a
report that would look exactly like the original form (Form 58XX (13262
- Draft, Tradi)? I guess I should refine that a bit, could I get Access
to fill in the correct county for instance and even the harvest date,
which is also a series of choice fields? I'm not looking for anyone to
do this, just tell me whether or not this is a dead end and if it can be
done, how I might proceed.
