It's not free, but John's latest book "Building Microsoft Access
Applications" has four complete Access applications, one of which is
for Registration Tracking and one which is for Membership Tracking.
I'm still working through this excellent book, it's on my desk right
now, but I'll bet you can modify one of those to fit your needs.
