Using X11 probably would be not a very big effort. I would more like to use Quartz directly. But as I wrote before, Mac users probably don't want MSEgui applications. I don't know if there are licensing problems, the Mac world is a closed world. AFAIK newer XCode versions don't support the gnu toolchain anymore.
