This changed with SP1. You can now have multiple SUPs within a single primary site just like you can have multiple MPs in a single primary site.
The WSUS instances for these SUPs can actually share the same DB also. Sharing the same content folder is next to useless though because the bulk of the content is stored in your source folders and is downloaded by ConfigMgr and not WSUS. If you have multiple, then they all have to be members nodes in the same NLB cluster farm. I believe you can only have up to 4 SUPs in a cluster farm and they have to share the same SUP database... It complicates things and adds additional considerations, such as downstream server catalog syncs on that low bandwidth connection.
