I think what you've described is by design. The app installation is automatic 
outside of their business hours or the user can go back into the App Catalog 
and initiate an immediate install.

So I have a bunch of applications advertised to users as available and requires 
permission
With the available option selected it is not possible to select the "allow 
following activities... outside maintenance window" because it is greyed out. 
Only Required updates have that option.

However once an application request is approved the user gets notified the app 
was approved by a notification window in the notification area.
When you go to software center to view or install the app the status says 
"waiting for the next available maintenance schedule"

I just did some testing and the install kicked off by itself once the machine 
was out of business hours, so it must be those maintenance windows that it sees.

Why would an optional selected application say it is waiting for a window?

it used to not do this, and now it does.
