Not sure if this is what you are seeing but... There was a problem with the Adobe Flash Player 12 ActiveX installation. The Flash Player installation was not writing a HKEY_LOCAL_MACHINE\SOFTWARE\Macromedia\FlashPlayer\SafeVersions\12.0 value. And that is what the installation rules were looking for, but since it doesn't exist, it always returned as "Failed". http://forums.adobe.com/message/6015648#6015648
To work around the problem, I modified the "Installable" and "Installed" rules for the update in SCUP to check the "Version" value under HKLM\Software\Macromedia\FlashPlayerActiveX.
