fyi, that blog and .hta was written when I was still using sms 2003. so forget
all that mof stuff--in CM12; you just go into default client agent, hardware
inventory, set classes… and browse to WMI on a box that ran this hta and go
find the class. obviously if you don't change the wmiclass stuff, it’ll be in
root\cimv2\sms, called “lease_info”--but I suspect you’ll likely change that…

From 2007:

I think one of the fields in there is a drop down. So feel free to use it as a
sample. Of course it writes to WMI, not the registry. Because that’s how I
roll.

Whatever tool you use, consider using drop downs for the responses. Otherwise
you might be getting some really off the wall answers if you allow straight
text.

Subject: [mssms] Simple User Questionnaire

Hello folks

I am working with a customer who needs to gather some info from their users to
pull into SCCM. What we are looking for is something which would allow us to
build a simple set of questions the answers to which we can write into the
registry.

Is there a good dummy's tool which folks use for this?

Jason
