All you need an Internet MP name, which should be resolved to DMZ MP/SUP/DP IP by clients while on internet. While on internet clients cannot talk to AD, so AD schema extension/publication is of no use for IBCM. During the client install for IBCM, you need to provide the internet name for MP/DP/SUP.
