EXPANDED BIOS documentation

When disassembling sources, I often encounter Extended BIOS calls (EXTBIO or
H.BEXT: #FFCA). Since there is no complete list with all device IDs (input
register D) and functions (input register E) I suggest we make one, complete
with explanation and input/output per function.
These are the device IDs currently known:
#00 - RS232 BIOS
#01 - RS232 BIOS.
#04 - DOS 2 mapper support routines.
#10 - Kanji Driver
#22 - Nestor TSRs (NestorAcentos, NestorCDPlayer, etc.)
#34 - MWMPLAY by Ramones (MoonBlaster 4 Wave replayer)
#4D - MemMan (Memory Manager)
#F0 - MGSDRV (SCC music-player of Multi-Mente)
Device IDs currently fully documented at the moment:
#04 Dos2 mapper support routines
and
#10 Kanji Driver
Please submit more docs or unlisted device IDs to this list.
