I'm having the same problem with the formatting, but i'm using a Schlumberger Cryptoflex 32k and a 72v2 reader, i've noticed that when i check the size of the card it keeps on decreasing, so perhaps there is a limit to how many times you can format, if you find out more about it, please let me know.
