The log output looks good, OCF and the Loader are working. The error occurs when the 'Initialize Update' command is issued. Just a guess, but problems like that are mostly timing problems. I had them with readers from Kobil. Switching to Towitoko's Microdrive solved most of those T=0-Protocol related problems ;) I would definitely recommend trying another reader, with 'no issues'.
