You need to load the musclecard applet on your card using the GemXpresso RADIII, then format it with XCardII. I know Ludovic did it so it should work.

You might have to add the ATR of your card to the Info.plist file located in:
/usr/local/pcsc/services/mscMuscleCard.bundle/Contents/

There should be a key called "spAtrValue" and you just need to add a string element with the ATR of your card:
