ISO/IEC 7816 part 4, except that its under ISO's copyright and they make you pay for every copy. However, they now publish on the web their drafts of new and revised smart card and related standards - but then they disappear as soon as the international ballot is completed. Some of them I then mount at www.iosis.org - I confess its quite a long way behind at the moment, but for an intro go there and download the first draft of the re-written 7816-4 (file 17n2060t.pdf). That rewrite is much easier to understand than the original.
