So how did folks use USB as a "host->reader packet bearer", before CCID?

If hosts send Gemplus chipset command format messages to the reader, over either TLP224 or GBP, and the command format wraps the host's actual ISO7816 TPDUs,

how (pre-CCID USB readers) did folks send the TLP/GBP block over USB?  as a HID report? using USB-RS232C translators?

In the CCID world, do any hostslibraries/drivers still construct GBP/TLP packets, and just tunnel these over the CCID endpoints?

Im hoping CCID compliance prevents proprietary frame/command tunneling.
