On the Mac, you need to use /System/Library/StartupItems/. You can see what is in these folders for a guidance. The .pkg installer on Musclecard does create a SmartCardServices folder. On Panther, the relevant resources already exist in /usr/libexec/SmartCardServices/StartupItem/ It might just be a question of symlinking the SmartCardServices to /System/Library/StartupItems/.
