FYI, I just talked to Sunil Vasudevan from Gemplus and he confirmed to me that the card I had sends a different ATR depending on whether it gets a soft or a hard reset. He said that the card was designed for a PC (not a GSM phone) and it is just a slightly different behavior that the muscle library must not be able to work with. He is going to send me some single ATR cards.
So anyway, I won't know for sure until I get the new cards, but I think the case is closed. Next time you hear some poor soul going through the type of problems I have been going through, you'll know what to tell them. This is definitely something that should be more well-documented. Everywhere I've looked it just says GemXpresso Pro 64K cards, so I just assumed that there were only one "flavor" of GemXpresso Pro 64K cards. But apparently there is more than one flavor and it is not obvious which one is the one that muscle knows how to work with. Another thing that might be worth considering is that the muscle library could be fixed to handle these types of cards. Thanks to all those who have helped with this, especially Dave.
