Purchase ISO/IEC 7816 Part 4 from www.iso.ch. Alternatively you can use the very slightly different version [1] from EMV www.emvco.com, and download for free the EMV card spec.

[1] At least, it was slightly different the last time that I looked at it - the differences were in the error handling and some parameter ranges of values, and neither 7816 nor EMV had a completely closed state diagram when I assessed it.
