I guess you are trying to format the card a second time with muscletool
an/or XCard. I thought, that it was obvious, that the '-setup' command
of the loader does exactly the same as 'format' does.
But the applet would throw a ISO7816.SW_INS_NOT_SUPPORTED, this should
be 0x6D00, not 0x6E00.
If -setup succeeds, then your card is ready to use. And just to avoid
another misunderstanding: 'Ready to use' means you may register it with
NSS as PKCS11 module and download a certificate from a CA.
