Look the code of pcsclite, it is enough simple.
But in my remember, IFDHCreateChannel is called as soon as the daemon is
started or if the reader was not present (e.g. an USB reader which was
not plugged) at starting when the reader is plugged.
You can also look the doxygen documentation (here a link to the function
which call IFDHCreateChannel [1]. look the function that reference it).
If you want you can also improve the documentation for the ifd handler
and describes the flow.
