You must use muscletools. www.musclecard.com. Hopefully it will be
linked. It has a format command.
muscleTool from [1] can do it, but you can actually personalise the
MuscleCard applet with a single APDU - e.g. a command like this in
GPShell:
send_apdu -sc 0 -APDU
B02A000038084D7573636C6530300401083030303030303030083030303030303030050208303030303030303008303030303030303000007800000201
