You should/shall use the PC/SC middleware to use your GnuPG card (see chapter 2.3.2. in the doc you are referring to). Mac OS X already provides the needed infrastructure for generic CCID readers and some non CCID readers. You can use the command testpcsc to check that your card reader is recognised by Mac OS X.
