I was having this problem with Mozilla 2.0 on Win XP. I got the libraries you 
asked for, updated my copies to merge in the changes between 1.1.5 and 1.1.6, 
and still couldn't make it work.

I eventually found the musclecard stuff (ohme's 2.0 source) was referring to 
the win 8.0 runtime libraries which for some reason couldn't be found for 
dynamic loading.    I recompiled everything to use static linkages for the run 
time and the problem went away. 

It may be worthwhile to check the linkages on the compiled packages and ensure 
each of the referenced modules is in the load search path.

Interestingly, Mozilla 2.0 will no longer load the Identity Alliance stuff.  
Something changed in the way mozilla is built or in the way the runtime 
libraries are found for VC 8.0...
