On OS X, I've often encountered the situation where there is no reader attached to the system but OS X kernel thinks there is a USB device connected. pcscd reports a reader etc. This does not depend on the reader model as it has happened with virtually any reader I have. I have given up trying to to extensive testing on OS X, as it is not too hard to make the securityd/pcscd/tokend stack crash and after securityd has crashed, the only thing to do is to reboot the computer.
