On OS X, I've often encountered the situation where there is no reader attached
to the system but OS X kernel thinks there is a USB device connected. pcscd
reports a reader etc. This does not depend on the reader model as it has
happened with virtually any reader I have.

I have given up trying to to extensive testing on OS X, as it is not too hard
to make the securityd/pcscd/tokend stack crash and after securityd has crashed,
the only thing to do is to reboot the computer.

If things stop working on OSX, do the thing suggested in the TV series "IT
crowd": "have you tried turning it off and on again?"
