I can't reproduce the problem with the readers I have here.
I have no problem with a KOBIL mIDentity XL for example.

Can you change the CCID driver Info.plist configuration file (should
be /usr/lib/pcsc/drivers/ifd-ccid.bundle/Contents/Info.plist) to
change ifdLogLevel from 0x0003 to 0x000F and try again.
With this change the driver will log all the USB exchanges.

Then send me the pcscd logs from /var/log/messages or wherever they
are stored on your system.
