Each call to SCardEstablishContext() should create a pcscd thread.
If the application do not call the corresponding SCardReleaseContext()
then you have a problem.

card driver needs to open 16 threads, or am I almost certainly facing a

Already tried, I really would like to, but I'm forced by a number of
dependencies that I could never resolve otherwise with the operating
systems we use for production.
