Sorry if I was unclear.  Those were two separate statements, one for a piece in 
the public domain, and one for a piece licensed under a creative commons 
license.  I've made a few more revisions here:

For a piece covered by creative commons license:

Translated into Clairnote music notation using LilyPond. The original LilyPond 
file is from the Mutopia Project, typeset in
standard music notation by Nikos Kouremenos (Mutopia-2014/03/24-519), used 
under a Creative Commons
Attribution-ShareAlike 4.0 International license, free to distribute, modify, 
and perform. http://creativecommons.org/licenses/ 

For a piece in the public domain:

Translated into Clairnote music notation using LilyPond. The original LilyPond 
file is from the Mutopia Project, typeset in
standard music notation and placed in the public domain by Stelios Samelis 
(Mutopia-2012/12/23-931), free to distribute, modify, and perform.


Hopefully that clears it up?  Let me know if you see ways to improve these.
