#3683: [patch] manual.{html,txt} doesn't contain -d option description

The -d option (available when compiled with --enable-debug) is not listed
in manual, but only in man page (see ticket
http://dev.mutt.org/trac/ticket/2830). The small attached patch adds the
description from man pages to the manual as well.
