Just FYI /var/tmp is wiped on every reboot on macOS.  I switched to
~/.cache/mutt because re-downloading the headers from a certain provider
can take several hours in which Mutt cannot be used for anything else.

And of course make it a directory to benefit from per-folder dbs.

That said I don't want to re-iterate or re-open the discussion, but I
think it would be helpful to designate header cache as something that's
more of a db thing than cache in user documentation, and to nudge
towards it being a directory instead of a file too.
