I don't know what you mean here. mutt is supposed to handle this situation just fine, see documentation for configuration variables smtp_url and smtp_pass. (That said, I currently have the luxury of a Linux desktop with an MTA, so I don't use this feature myself.) What happens if you just set these variables accordingly? Details, please. As plan B, it is easy to set up Exim to relay all external mail through a gateway. Look at chapters 16 (The accept router) and 30 (The smtp transport) of the Exim specification [1].
