What do you mean with “inline expansion of articles”? Do you mean
transclusion of articles with the {{:Some article}} syntax? And if
that doesn’t work, can you provide a minimal example?

What’s in the PDF? Does it contain the specified article “Main Page”?

Maybe I just don’t get what your problem is.
How did you add the pages? In what context are the images shown (and
where do they come from) if there are no actual pages in the PDF? How
do trigger the PDF generation: via the “Download as PDF” link or from
an article collection via the download box on the Special:Book page?
Can you mail me an example PDF (not to the list but to me directly)?
