I just noticed that we are missing the top level in the PDF TOC:

For example I have:
=General Strategy=
blah
==Stack to Registers==
xxx
==Exception Handling==
zzz
=Java Bytecode=
blah blah
==aaload==
===Description===
...

And the resulting TOC is:
Doc/Page Name
+Stack to Registers
+Exception Handling
+aaload
++Description
....

Notice that the single equal level 1 entries are not shown in the TOC.
Any reasons why?

There was indeed a reason for skipping lvl 1 headings from the PDF bookmarks. Level 1 headings are the same level as articles - therefore in a way they should not be used inside an article at all (at least in my opinion).
But who am I to tell people how to structure their mediawiki articles.
