I assume you are talking about the printed books.
For the books cover image you can only use images that are part of the book. Therefore you need to include the image in an article that is part of the book. If you don't really want the image to appear inside the book you could try to use a dirty trick: include the image somewhere with a width of 1px. Take a look at the fantastic example at http://simple.pediapress.com/wiki/MarkupReference/Images#inline (section "inline" at the very bottom currently)
