I have a wiki page that only makes use of 4th level headings with text between them. However when looking at the PDF index tree in my PDF viewer, they are not displayed on the same level - in fact they are nested. The second heading is a child of the first, the third a child of the second, and the rest of the headings are a child of the third heading. When replacing all headings by 3rd level headings, the same happens, just with one hiearchy less (all following headings are a child of the second heading). Is this a known issue?
