I have that problem as well.  I keep sync to "Sync on connect" because of this.

Steps to reproduce:

New file, sync continuously to PPC, dock PPC
Create a new task
Task gets synced
In text field, type "1" and move focus out of Task Notes (*).  Move focus back 
to Task Notes.
Wait for "data transferred to remote storage"
(Quickly) type "2" in Task Notes.
MLOPPC says "file is being synced"
MLOPPC is done
MLOPPC says "file is being synced"
MLOPPC is done
MLO says "Receiving remote data.  Please wait..."
You will get a "Resolve Sync Conflict" 
In the conflict screen, the desktop says "12" and the PPC says "1".

I believe you need to be modifying an item that is simultaneously being updated 
(ie: typing in the notes field).  If you move from item to item, there will not 
be any problem.

For example, this is ok:  create tasks A through J.  Complete "A", wait for 
"data transferred to remote storage", complete "B".  Everything will sync, and 
you can keep completing tasks without encountering any conflicts.  You can also 
edit the notes in sequence as long as you don't modify during an update.

(*) Another bug report:  If you don't move focus out of Task Notes, MLOPPCSync 
will not detect the change (and the "Notebook" icon does not appear, either).

I use the "Sync continuously" feature to sync desktop <-> PocketPC.
While the PocketPC is connected, if I modify something on the Desktop
MLO it correctly the PocketPC MLO.
The bug/problem is that if I modify other things while it's still
connected, every time the Desktop tries to update the PocketPC it find
a unexistant conflict like if each update/modification to the PocketPC
MLO were done on the PocketPC (I know this because I see that the date
on the PocketPC part is (uncorrenctly) that of the last sync), when in
truth I did the modifications on the Desktop.
