If you have "Hide completed tasks" (top left corner) selected and all the subtasks of a folder are completed the +/- sign will disappear as there is nothing to expand/view. You can test if this is the issue by unselecting it. Otherwise, I suspect this is a problem particular to your setup, and would suggest reinstalling MLO.
