I’m not sure of the details but I know that my inbox has the oldest stuff in the middle with some new items at the top and some at the bottom. I agree that the correct process is always to add to the bottom.

When I'm planning, sometimes I would miss some subtasks that should appear in front, in the middle or in the end of the list. I would add the missed subtask at the proper insertion point like the way MLO is working now. I wouldn't want it to always go to the bottom especially if I have a long list of subtask only to move the inserted subtask way to the top. This is one of my problems with RightNote but it is entirely different topic so I'll stop here.
