Leonard, What command are you using to start the slave server? I was thinking maybe it wasn't reading the correct configuration file?
This one is pretty strange. Have you thought about upgrading the slave mysql version, just to disregard that it might be a problem between the two versions? One thing, in your user table, are you using the hostname of the slave server to specify where it's coming from? ( I know you said you did % already, so this shouldn't matter.. but.. who knows? ).
